Treatment Approach

The treatment of breast cancer can usually depend from person to person who is suffering with it. Usually depends what is the stage of cancer the person is suffering from , the hormone receptor status and the persons overall health . The plan of treatment that has to go about often combines various approches in order to receive the best results and care .

Surgical Treatments

Breast conserving surgery

Only the tumor is removed from the breasts in this process(Lumpectomy and Quadrantectomy can also be included).

Features:

  • Maintains the appearance of the breasts.
  • Fast recovery period.
  • Further process followed by radiation.
  • Has a sucess rate of 90%.

Oncoplastic Surgery

Method which involves the removal of the tumor and plastic surgery performed in order to maintain the shape of the breasts

Features:

  • Helpful for removal of large tumors.
  • Has a sucess rate of around 90%.
  • Radical mastectomy
  • Further process may be required if clear margins are not achieved.

Lymph Node Surgery

Surgical process through which the lymph nodes are removed to monitor the spread of cancer and stop the further metastases.

Procedures:

  • Sentinel lymph node biopsy
  • Axillary lymph node dissection
  • Z0011 Trial

Radiation Therapy

Regional Node Radiation

Technique used to manage the axillary nodes for the patients who are suffering from stage 2 and stage 3 breast cancer.

The duration of treatment can go upto 7 weeks. The radiation session has to be taken 5 times a week .

Radiation After BCS

Treatment with BCS and radiation which is comparable to mastectomy(Non breast Conserving Surgery) has proven to shown 90-95% local control at 10 years.

The duration of treatment can go upto 5-7 weeks or even 3 weeks for some patients. The radiation session has to be taken 5 days a week.

Hypofractionated Whole-Breast Irradiation Therapy

Higher amount but lesser doses are used to remove the tumor from the breasts. Due to this the duration of the treatmnent is reduced considerably.

Treatment can go on from 3 - 5 weeks. The therapy session has to be taken once a week.

Chemotherapy

Adjuvant Chemotherapy

The chances of renewal for high risk cancers are reduced. The drugs which can be given for treatment purposes:

Anthracyclines

Adriamycin,Doxorubicin

Antimetabolites

5-FU , Methotrexate

Alkylating Agents

Cyclophosphamide

Neo adjuvant Therapy

For the purpose of breast conserving surgery to take place neoadjuvant therapies are adviced to the patients, in this the large tumors are shrinked. The antibiotics which can be given or adviced to the patients:

HER2-Targeted

Trastuzumab, Pertuzumab

CDK4/6 Inhibitors

Palbociclib, Ribociclib

PARP Inhibitors

Olaparib, Talazoparib

Upcoming Treatment Options

SERMs Plasminogen activator Inhibitor-1 and Urokinase Plasminogen activator

Low levels of these markers are used in newly diagnosed breast cancer which indicates low chances of reccurence .

Oncotype DX assays

By measuring the expression of 21 genes it helps to determine which patients are less likely to get the benefit of chemotherapy.

Molecular Profiling

Method which helps to get the predictive data regarding individual tumor will help to clinical use of biological therapies and help to get on a conclusion which treatment method has to be chosen.
